This form is completed by the Advisor once they assess their student's clinical case study as meriting Content Approval.

As a reminder, the Doctoral Project Committee (DPC) reviews CCS manuscripts for Content Approval once the Advisor has assessed the manuscript as finished. 

Please use this checklist to evaluate the manuscript’s readiness for submission to the DPC for Content Approval, then submit the student’s manuscript along with this completed form.
