Transformative Learning Lab Attendance Release Form

Responsibility:

By signing below, the student certifies that they do not have any mental, physical or other condition that would create a hazard for themselves or other students during the Transformative Learning Lab (TLL). The student further agrees to wear sturdy, ankle- and foot-protecting footwear, such as hiking boots, wear clothing appropriate to the context of the Lab, and take full responsibility for the prevention and treatment of any medical conditions that may arise for them during the Lab. The student further confirms their understanding that the University is not providing medical or psychiatric care prior to, during, or after the Lab. Further the student understands that the University is not providing security services, travel agency services, transportation, lodging, meals, country, city or event-site entry fees, or concierge advice related to the Lab host city.

Governing Documents:

The student understands and agrees that the terms of their Enrollment Agreement at Meridian University as well as the requirements of the University’s Student Code of Conduct apply to the TLL.

Medical Conditions & Injuries:

By signing this agreement, the student confirms that they are of sound physical and mental health and will participate only voluntarily in psychologically and physically challenging activities.

Statement of Release:

The undersigned student understands that the University is in no way responsible and can assume no liability of any nature whatsoever for the student’s experiences prior to, during, or subsequent to the TLL. The undersigned is aware that a TLL could involve the risk of personal injury and damage or loss of property, like any other travel or event could. In consideration of the benefits to be derived from participation in the TLL, the undersigned voluntarily accepts all risk of personal injury and property damage or loss arising from their participation and hereby agrees that they and their dependents, heirs, executors and assigns, do release and hold harmless Meridian University and its employees, officers, directors, trustees or representatives from any and all claims, including claims of illness, bodily injury, death or property damage or loss, however caused, arising from or related to the TLL.

The undersigned has carefully read this agreement and affirms that they are not relying on any oral or any other written representation, other than this document, as a basis for executing this Release.
